IMPORTANT- If the name on the card used below is different than the client name, the client is using a third party for payment to Pathways Counseling. By using a third party to pay fees in part or in full, the client consents to the following: 1) Pathways Counseling will release necessary billing information, including your name, session fee, date, times, and number of sessions the third party paid for to the named Third-Party Payer upon request. 2) Any records outside of necessary billing information will NOT be released to the Third-Party Payer. 3) It is the client’s responsibility to alert the third party payer regarding charges related to late cancellations or no shows. The client is personally responsible for any fees Pathways Counseling is unable to recover from the third party. This means if the third party chooses for some reason not pay Pathways Counseling, the client still responsible for all outstanding fees.
